SMS & Messaging Consent Policy

South Lakeland Dental Management LLC ("we," "us," "our," or "South Lakeland Dental") offers SMS and text messaging programs to provide patients and prospective patients with appointment reminders, follow-ups, and promotional offers related to our dental services. This SMS & Messaging Consent Policy ("Policy") describes how we collect consent to send text messages, what types of messages you may receive, how to opt out, and your rights under applicable law, including the Telephone Consumer Protection Act (TCPA) and guidelines established by the Cellular Telecommunications Industry Association (CTIA).

We operate two distinct SMS messaging programs. You may opt in to either or both at the time you provide your phone number:

Promotional / Marketing Messages

Special offers, discounts, service announcements, and marketing communications about South Lakeland Dental services and promotions.

Transactional / Informational Messages

Appointment reminders, appointment confirmations, follow-up care instructions, scheduling updates, and other service-related communications necessary to your care and relationship with our practice.

3. How You Provide Consent (Opt-In)

You may provide your express written consent to receive SMS messages from South Lakeland Dental in any of the following ways:

Web Form Submission: By completing the contact or appointment request form on our website at offer.southlakelanddental.com/contact and affirmatively checking the applicable consent checkbox(es) for Promotional and/or Transactional messages before submitting the form.

In-Office Intake: By providing your phone number and signing a written consent form at our office located at 4912 South Fork Dr., Lakeland, FL 33813.

Keyword Opt-In: By texting the keyword START to one of our designated phone numbers.

Promotional messages: Message frequency varies. You may receive up to 8 marketing messages per month, though frequency will depend on active offers and campaigns.

Transactional messages: Message frequency varies. Messages are typically sent in connection with upcoming appointments, post-visit follow-ups, and scheduling changes.

5. Message & Data Rates

Standard message and data rates may apply to any messages sent or received, depending on your mobile carrier and plan. South Lakeland Dental is not responsible for any charges imposed by your wireless provider. Contact your carrier for details about your messaging plan.

6. How to Opt Out (STOP)

You may opt out of receiving SMS messages from South Lakeland Dental at any time by using any of the following methods:

Reply STOP: Text the word STOP to any message you receive from us. You will receive a single confirmation message acknowledging your opt-out request, and no further messages will be sent.

Call Us: Call our office at (863) 238-2521 and request removal from our messaging program.

Email Us: Send a request to [email protected] with the subject line "SMS Opt-Out" and include the phone number you would like removed.

Opt-out requests are processed immediately. After opting out, you may still receive a single confirmation message. Opting out of promotional messages does not automatically opt you out of transactional messages, and vice versa. Reply STOP to each program's messages to opt out of both, or contact us to opt out of all messaging at once.

8. Supported Carriers

Our SMS programs are compatible with all major U.S. wireless carriers including, but not limited to: AT&T, Verizon, T-Mobile, Sprint, U.S. Cellular, and their affiliated MVNOs. Carrier support may change without notice. Service may not be available on all carriers. T-Mobile is not liable for delayed or undelivered messages.

9. Privacy & Data Protection

We respect your privacy. Information collected in connection with our SMS programs is handled in accordance with our Privacy Policy.

Phone numbers collected for SMS messaging are used solely to deliver the messages described in this Policy.

We do not sell, rent, lease, or share your phone number or opt-in information with third parties for their own marketing purposes.

We may share your information with our messaging platform provider (Twilio) solely for the purpose of delivering messages on our behalf, and with service providers who assist us in operating our practice, subject to confidentiality obligations.

We implement commercially reasonable security measures to protect your personal information, including your phone number and consent records.

10. Consent Is Not Required for Service

Your consent to receive SMS messages is entirely voluntary. Opting in to promotional or transactional text messages is not a condition of scheduling an appointment, receiving dental care, making a purchase, or engaging with South Lakeland Dental in any capacity. You may opt out at any time without any impact on your ability to receive our services.

11. Age Restriction

Our SMS messaging programs are intended for individuals 18 years of age and older. If you are under 18, you must have the consent of a parent or legal guardian to opt in to our messaging programs. We do not knowingly collect phone numbers from individuals under the age of 13.

12. Disclaimer of Liability

South Lakeland Dental and its messaging platform providers are not responsible for:

Delayed, undelivered, or misdirected messages due to carrier network issues, device incompatibility, or technical failures beyond our control.

Data charges or fees assessed by your mobile carrier in connection with messages sent or received.

Any errors in information provided by you, including incorrect phone numbers.

13. Changes to This Policy

We reserve the right to update this SMS & Messaging Consent Policy at any time. Changes will be posted on this page with a revised "Last Updated" date. Your continued participation in our messaging program after changes are posted constitutes your acceptance of the revised Policy. We encourage you to review this page periodically.
